This form is to be used ONLY by individuals who have been issued a NEW waiverable traffic citation, where the initial Court date is on or after APRIL 1, 2025. “Waiverable” refers to citations that can be paid without requiring a Court appearance.
To be considered for a payment plan, you need to complete all the requested information. Submitting this request does NOT guarantee approval. Upon receiving this form, the Court will review your eligibility and, if you qualify, will send the necessary documents to you via e-mail. Furthermore, if you enter into a payment agreement, you will be making an admission of guilt to the offense(s) listed on your citation. If it’s a traffic citation, a record of that conviction will be sent to the Bureau of Motor Vehicles, which may assess points to your license. Approved applicants must pay all fines and Court costs in full by the timeframe established by the Court in the documentation that will be sent to you.
